1095 Employee Communication

The Affordable Care Act (ACA) requires all Applicable Large Employers (ALE) with 50 or more FTE employees to distribute a 1095-C to all full-time employees by February 1, 2016 (extended to March 31, 2016. The links below contain an employee communication about the form(s) as well as a cover letter that you may want to include with the form, though not required. Please note that there are different documents depending on whether your plan is fully-insured or self-insured.
